CMS-1500 Claim Form Exports

How to create, review, and download CMS-1500 insurance claim PDFs from PracticeRunner invoices.

PracticeRunner can help you prepare CMS-1500 claim form PDFs from invoices. This is a claim export workflow, not electronic claim submission. You review the claim, download the PDF, and then submit it through the payer portal, clearinghouse, mail process, or other workflow your practice uses.

CMS-1500 claim exports are designed for psychotherapy and other insurance-billable clinical services. They are hidden by default for consultation cases, where diagnosis, insurance claims, treatment plans, and superbill workflows do not usually apply.

Turn On Claim Forms

  1. Open Settings -> Billing.
  2. Turn on Use insurance claim forms.
  3. Save billing settings.

When claim forms are enabled, invoices show a Create claim action. Provider profiles also show taxonomy code fields when they are relevant for CMS-1500 claims.

Set Up Claim Details

Before creating claims, review the information that payers usually expect:

  • client demographics and address
  • client insurance member ID, group number, payer name, and plan name
  • active diagnoses for the client
  • service codes, dates, fees, and appointment details on the invoice
  • billing provider name, NPI, tax ID, address, and taxonomy code
  • rendering provider NPI and taxonomy code when the rendering provider differs from the billing provider

PracticeRunner builds a claim snapshot from invoice, client, insurance, diagnosis, provider, and practice settings. Editing the claim review page changes the claim snapshot. It does not automatically rewrite the source client or invoice data unless you explicitly choose an available update option.

Create a Claim From an Invoice

  1. Open Billing.
  2. Open the invoice you want to use.
  3. Select Create claim.
  4. Review the claim form page.

If a claim already exists for that invoice, the button opens the existing claim instead of creating a duplicate.

Review the Claim

The claim review page shows the claim values PracticeRunner will place on the CMS-1500 form. Review any validation warnings before downloading. Required missing fields block export because the claim would be incomplete.

Use the review page to confirm payer-specific details such as:

  • insured information
  • diagnosis pointers
  • provider and service facility details
  • assignment setting
  • patient account number
  • line-item charges

Payers can vary in what they require. Confirm payer rules before sending a claim outside PracticeRunner.

Download the PDF

PracticeRunner supports two CMS-1500 PDF modes:

  • Download full claim form PDF: includes the generated CMS-1500 form layout and the claim values. Use this for review, records, downloads, and payer portals that accept uploaded PDFs.
  • Download fields-only PDF for pre-printed CMS-1500 forms: prints only the claim values on the CMS-1500 coordinate plane. Use this when printing onto official pre-printed CMS-1500 stock.

After export, PracticeRunner marks the claim as exported and records the action in the audit trail.

What This Does Not Do

CMS-1500 export does not submit claims electronically, check payer eligibility, receive ERAs, post insurance payments, or manage claim adjudication. It prepares a reviewed claim PDF so your practice can submit it through the external claim workflow you already use.
